COVID-19 Vaccine consent form

Individuals may download the COVID-19 vaccine consent form and complete it in advance of coming to receive the vaccine or a booster dose.

Current COVID-19 safety measures at KRHC

  • The Respiratory Care Area at KRHC is located on the east side of the clinic, using a seperate entrance. Appointments are required for this area - please contact the clinic to schedule your visit.
  • If you are coming to the clinic for an appointment, you may choose to register via telephone from your vehicle or enter the clinic lobby to register and wait for your appointment.
  • If you are experiencing COVID-19 symptoms, please call before entering the KRHC facility for an appointment or to receive care. This allows KRHC staff to direct you appropriately. 

Visitor Restrictions at KRHC

  • There are currently no visitor restrictions for inpatients without COVID-19.
  • COVID-19 patients may have one designated visitor each day. That visitor must be 18 years or older and healthy with no symptoms of illness, including respiratory or fever. Visitors must not be under quarantine or have a current positive case of COVID-19.
  • Guests are asked to follow all current safety guidelines during their visit.

COVID-19 Testing at KRHC

  • COVID-19 testing is available through the Kossuth Regional Health Center clinic. Please call 515-295-2451 to schedule an appointment for your COVID-19 test. 
  • Take Home COVID-19 tests are available in Community Health by calling 515-295-4430. Each test kit includes step-by-step instructions including how to activate the test kit, collect the test sample, return sample to the State Hygienic Lab and when to expect results. Kossuth Regional Health Center would also like to remind patients not to visit the Emergency Room solely for a COVID-19 test after hours. Unless there is a medical emergency, the KRHC Emergency Department will not perform outpatient COVID-19 testing.
