Community Health- Immunizations

Please replace the text at the very top with the following:  Community Health provides child and adolescent immunizations throughout Kossuth County for newborns through 18 years of age.  In flu season Community Health provides the flu shot and FluMist for all ages, including adults.  Pneumonia vaccine is available at flu clinics for those ages 65 and older who have never had a pneumonia shot.

Vaccines for Children
(Federally funded immunization program)

Community Health is required to abide by certain requirements in order to give children and adolescents their vaccines. These requirements are as follows:

  • Vaccine available to newborns to those 18 years of age 
  • Medicaid/Title 19 enrolled or
  • No health insurance or
  • American Indian or Alaskan Native or
  • Underinsured (no immunization coverage, covers only certain vaccines or caps the cost at a certain amount)
  • Unmet deductibles or co-pay do NOT qualify persons for Vaccines for Children

If you have insurance, please contact your insurance company to determine if your insurance covers immunizations or includes them under the category of preventative care.  If insurance will pay, please call your health care provider's office to receive vaccinations.  If your insurance doesn't cover immunizations or for more information about immunizations, please contact Community Health at 515-295-4430 or 1-800-603-8433 to arrange a time for your child to receive immunizations.
