Case Management

Mercy Medical Center - North Iowa has located Chuck Redemske, RN and Brenda Kern, ARNP, Professional Nurse Case Managers, at Kossuth Regional Health Center to provide day to day management of patients with complex chronic illnesses who demonstrate a high utilization of health care resources.

Services:

• Monitor health status through the development of long-term relationships
• Assist with medication management
• Access community and home care services that benefit patients
• Discuss and help individuals make choices regarding the end states of life
• Provide direct nursing care
• Prevent duplication of services
• Evaluate proper health resources and make referrals
